This page adds:

• name
• address
• email / phone
• birthdate
• insurance details
• General Practitioner details (including contact details)

NOTE: address, insurance details and GP details are available online via the Vecozo COV link (for the netherlands). This page should have the option (interface) to automatically fetch additional details (implementation = connect to vecozo) and add them to the form details.
